To Fax/Mail your Checklist, just:

• Download the checklist from the links below for Volume-Submitter and Individually Designed plans. You must have Adobe Acrobat Reader version 5.0 or higher installed in order to access these checklists.
• Print the checklist from your computer.
• Complete the checklist or Adoption Agreement (you must contact the Plan Sponsor for copies of the Adoption Agreement, and plan document if you do not have them).
• Include any language modifications (with help from one of our document consultants, if needed).
• Request a proof of your SPD Booklets on the same form!
• Fax or mail your Checklist or Adoption Agreement to

Using an IDP/Volume Submitter Checklist/Transmittal Form you can order a Plan and Trust document, supporting forms package, SPD, and all notice, election, and IRS submission forms.

Prototype and Prototype-Formatted Volume Submitter
In order to process a Prototype or prototype-formatted Volume Submitter Supporting Forms Package, you must have purchased the corresponding Plan document and received an IRS Opinion or Acceptance letter for the specific document you wish to process (e.g., Standardized Profit Sharing). You must provide us with a completed Transmittal/Order Form and a completed Checklist Addendum (see below). In addition, you must submit a completed Adoption Agreement. You received blank Adoption Agreements when you purchased the Prototype Plan document or the prototype-formatted Volume Submitter Plan document.
