  • Print the checklist from your computer.
  • Complete the checklist or Adoption Agreement (you must contact the Plan Sponsor for copies of the Adoption Agreement, and plan document if you do not have them).
  • Include any language modifications (with help from one of our document consultants, if needed).
  • Request a proof of your SPD Booklets on the same form!

Prototype and Prototype-Formatted Volume Submitter
In order to process a Prototype Supporting Forms Package, you must have purchased the corresponding Prototype Plan document. You must provide us with a completed Transmittal Form and a completed Checklist Addendum (see below). In addition, you must submit a completed Adoption Agreement. You received blank Adoption Agreements when you purchased the Prototype Plan document or the prototype-formatted Volume Submitter Plan document.
 
Note: The prototype-formatted Volume Submitter documents listed in this section are Plans that include a separate Plan and prototype-formatted Volume Submitter Adoption Agreements. You must have purchased these prototype-formatted Volume Submitter Plan documents to order the Supporting forms package.
